Required Enrollment Documents

REQUIRED ENROLLMENT DOCUMENTS

 

  • Birth Certificate/I-94 Refugee Card
  • Complete Immunization Record and Physical Examination documentation
  • All prior school records
  • Proof of Residency:

 

PROOF OF RESIDENCY

All applicants must submit at least one document from each of the following columns:

Column C

  • Recent mortgage payment
  • Copy of Lease and copy of recent rental payment
  • Legal affidavit from landlord affirming tenant and record of recent rental payment
  • Section 8 Agreement

A utility bill or work order dated within the past 60 days showing Westfield address:

  • Gas bill
  • Oil bill
  • Electric bill
  • Cable bill
  • Telephone bill
  • Current vehicle registration
  • Excise (vehicle) Tax bill
  • Property Tax bill
  • Bank Statement
  • Credit Card Statement
  • W-2 Form
  • Valid driver’s license with current address
  • Valid photo ID card with current address
  • Valid Passport

Legal guardianship requires additional documentation from a court or agency.

The documents will also be required for any change of address.

 

**All documents must be submitted by appointment to the Central Registration Office located at 94 North Elm Street to complete your student’s enrollment into the Westfield Public Schools.
